Physical Health & Wellbeing Navigator, Open Mental Health

Location - Somerset

Open Mental Health are looking for a Physical Health & Wellbeing Navigator to support those with severe mental illness to attend physical health checks and access services.

Open Mental Health (OMH) are an alliance of mental health organisations who have come together to provide 24/7 support to adults in Somerset, with a shared ambition to ensure that people living with mental health problems get the right support at the right time. Working together, they support people to live a full life by enabling access to specialist mental health services, housing support, debt and employment advice, volunteering opportunities and more, helping to support and improve their wellbeing and quality of life.

Part of this support includes their Community & Outreach Support team, who provide time limited, person centred support to people with a severe mental illness (SMI) or personality disorder, working with them before during and/or after the SMI physical health check.

OMH are looking for a Physical Health & Wellbeing Navigator to join this team and help explore the barriers an individual might experience when accessing services, address any concerns that might arise about the physical health check itself, and support them to explore ways they can improve their general health and wellbeing.

The successful candidate will have lived experience of mental illness, and/or a good understanding of mental illness and the impact it can have on someone. They will be able to access, plan and monitor support to acheive and evidence recovery and service outcomes. They will also have experience of partnership working, negotiation and liaison work with other agencies.

This is a part time role, working 21 hours per week on a 12-month contract. The salary is £22, 004-£26,053 per annum (pro rata), depending on experience.
