The West Mendip Primary Care Network are looking for a Health Connector to engage with the community and patients in the Glastonbury area.
The West Mendip Primary Care Network (PCN) is a collaboration of five surgeries in Glastonbury and Wells. It is one of 13 similar PCNs in Somerset that allows surgeries to work together at scale, providing a wider range of services to their patients.
Health Connectors work within these PCNs, offering one-to-one support or signposting patients into local services in their communities that can support them to better manage their health, wellbeing and social welfare. They also run other local services such as Talking Cafes, healthy lifestyle programmes or pain management groups.
The West Mendip PCN are looking for a Health Connector to deliver person-centred care and support. They will have a particular focus on supporting patients and community engagement in the Glastobury area, primarily from the Glastonbury Health Centre.
The successful candidate will have an understanding of the needs of people with long-term health conditions, and knowledge of the personalised care approach. They will be able to actively listen, empathise with people, and support others in a way that inspires trust and confidence. Knowledge of VCFSE and community services in the area would be an advantage.
The role is 30-37.5 hours per week on a permanent contract. The annual salary is £25,420 (FTE).
